Clinical Pharmacist

Job Type Full-time Description The Clinical Pharmacist ensures the safe and effective use of medications by providing comprehensive clinical pharmacy services, including medication management, patient education, and collaboration with healthcare professionals to enhance patient care. This role prioritizes medication safety, evidence-based practice, and quality improvement while supporting optimal patient outcomes. Principal Activities & Responsibilities Accurately prepares, verifies, and dispenses medications in compliance with prescription orders, TRHC protocols, and regulatory standards. Provides face-to-face, telephonic, or virtual pharmaceutical care services to patients, promoting safe, effective, and cost-conscious medication use. Initiates, modifies, and manages medication therapy within established clinical protocols and scope of practice, in coordination with clinic providers. Reviews medication orders for appropriateness, including indication, dosing, contraindications, and potential drug interactions. Orders and interprets laboratory tests as appropriate for medication monitoring and patient safety, in coordination with clinic providers. Manages medication refills, including withholding, modifying, or escalating therapy when monitoring requirements are not met or safety concerns are identified, in coordination with clinic providers. Counsels patients and their families on proper medication use, side effects, and adherence strategies. Collaborates with physicians, behavioral health, nursing, and other healthcare team members to optimize patient outcomes. Participates in the development and implementation of clinical protocols, pathways, and evidence-based guidelines. Monitors patient outcomes and evaluates the effectiveness of therapy, adjusting treatment as appropriate. Maintains accurate and timely documentation of clinical interventions and patient encounters in the electronic health record. Engages in quality improvement initiatives focused on medication safety, adherence, and clinical outcomes. Stays current with advancements in pharmacotherapy through ongoing education and training. Assists in managing pharmacy inventory and identifying cost-effective medication strategies. Collaborates efficiently and effectively while consistently demonstrating professionalism and maintaining positive, respectful relationships with internal teams, external partners, and Tribal members. Other duties as directed by management. Level of Authority & Restrictions Exercises independent clinical judgment in medication management while collaborating with healthcare teams. Functions within established clinical protocols, guidelines, and regulatory standards; coordinating care with clinic providers and escalating complex or high‑risk cases as appropriate. Physical & Mental Demands Must be able to walk, talk, hear, use hands to handle, feel or operate objects, tools, or controls, and reach with hands and arms. Vision abilities required by this job include close vision and the ability to adjust focus. May be required to push, pull, lift, and/or carry up to 30 pounds. Must be able to stand, walk, reach with hands and arms, and climb or balance. Must be able to sit and type/work on a computer. Must be able to stand for long periods of time. Working Conditions & Environment May require working occasional nights and/or weekends. Moderate noise level with frequent interruptions and distractions. Must be willing and able to travel both locally and within the CTCLUSI service delivery area and work at locations other than Three Rivers Health Center. Requirements Must be 18 years of age or older. A Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy. Minimum of two (2) years of clinical pharmacy experience or completion of a postgraduate pharmacy residency (PGY1). Experience and proficiency in the use of Microsoft products (Excel, Outlook, PowerPoint, Word, etc). Demonstrates comprehensive knowledge of pharmacotherapy, medication safety principles, and applicable regulatory requirements. Applies sound clinical judgment to ensure patient safety, including identifying and managing medication-related risks, contraindications, and potential drug interactions. Utilizes strong clinical judgment and analytical skills to assess patient needs and optimize therapeutic outcomes. Excellent verbal and written communication skills to effectively counsel patients and collaborate with healthcare professionals. Proficient in using electron health records (HER) and pharmacy information systems for documentation and medication management. Ability to prioritize tasks, manage time effectively, and adapt to changing clinical demands in a fast‑paced environment. Ability to communicate clearly and effectively in English, verbally, in writing or by other acceptable means. This position is considered a covered role per the CTCLUSI Background Investigations Policy. A state criminal background check and fingerprint‑based background check will be required as a condition of employment. This position is designated as safety‑sensitive and is subject to pre‑employment and other authorized drug and alcohol testing in accordance with company policy. Please note that the use of marijuana is prohibited for employees in this position, regardless of state legalization status. Must have employment eligibility in the U.S. Indian preference will be observed in the hiring process.
