Physical Therapist (PT) - Home Health

We are seeking a dedicated and skilled Physical Therapist (PT) for a home health role in the Mt. Angel, OR area and surrounding regions including Woodburn, Aurora, Molalla, Gervais, and Silverton. The ideal candidate will have experience in the home health setting and be ready to provide one-on-one patient care.

Key Responsibilities

• Deliver high-quality physical therapy services to patients in their home environment.
• Conduct patient evaluations and develop individualized treatment plans.
• Manage a case load of up to 6 patients per day, ensuring adherence to productivity standards.
• Utilize OASIS for patient charting and documentation.
• Collaborate with other healthcare professionals to optimize patient outcomes.

Shift Details

• Standard Shift: 8-hour days, Monday through Friday (08:30 - 17:00).
• Block scheduling and weekend work are not required.

Requirements

• Minimum of 1 year of skilled home health experience is required.
• Current Basic Cardiac Life Support (BCLS) certification.
• Must have a valid physical therapy license (pending licenses are accepted).
• Ability to travel with personal transportation is necessary.
• COVID-19 vaccination series and one booster is required.

Additional Information

• Facility allows cancellation of up to one shift per pay period.
• Drive time may occasionally reach a total of 60-90 minutes per day.
• Business casual attire is preferred; scrubs are allowed but not required.
• Free parking available and mileage reimbursement at the IRS standard rate.
