Outpatient Primary Care Nurse Practitioner or Physician Assistant

Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to provide patient-centered care in our multispecialty practices across New York’s Capital Region. In this role, you will evaluate and treat diverse patients, manage acute and chronic conditions, order and interpret diagnostics, prescribe medications, and deliver preventive and wellness-focused care. You’ll work closely with physicians, specialists, and support staff in a collaborative, physician-owned group that values quality, work–life balance, and long-term relationships with patients and colleagues.
