• Active Registered Nurse license
• Knowledge of hospice care, Medicare, and Medicaid guidelines.
• Strong assessment, documentation, and communication skills.
• Ability to work independently in home-care environments.
• Experience with IV therapy and venipuncture preferred.
• Valid driver’s license, reliable vehicle, and insurance.
• Ability to meet physical demands of home-based patient care.
• Commitment to hospice philosophy and ethical nursing standards.

• Respond to after-hours patient/family calls and assess needs.
• Provide pain and symptom management per physician plan of care.
• Perform ongoing patient assessments and document care accurately.
• Communicate with physicians and interdisciplinary team members.
• Educate and support patients and caregivers.
• Coordinate referrals to hospice disciplines and community resources.
• Maintain confidentiality and comply with agency policies.
• Work independently and prioritize urgent situations.
