Job Summary:
Homeside Rehab, a premier home health care provider, is seeking a compassionate and skilled Occupational Therapist or Occupational Therapy Assistant (OTA) to join our home health team. As an Occupational Therapist or OTA, you will play a vital role in helping patients reach their full potential by delivering individualized, evidence-based therapy in their own homes.

Job Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical assessment and patient goals

• Provide evidence-based OT interventions to improve motor skills, functional independence, and daily living outcomes

• Maintain accurate, timely documentation of all patient care in accordance with regulatory requirements

• Collaborate with the clinical team to ensure coordinated and effective treatment

• Communicate clearly with office staff, patients, and caregivers regarding progress and care plans

• Educate patients and caregivers on strategies to support continued progress between sessions

• Adapt treatment approaches to meet the needs of patients with diverse diagnoses, ages, and functional levels

• Travel to patients' homes within the assigned service area, maintaining a flexible and reliable schedule

Job Qualifications:

• Active NYS Occupational Therapy License (OT or COTA) required

• Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program (OT), or completion of an accredited OTA program (COTA)

• Prior home health experience preferred; new graduates are welcome to apply

• Basic computer proficiency for electronic documentation

• Valid driver's license and reliable transportation

• Strong communication and interpersonal skills, with the ability to build rapport with patients and families

• Ability to work independently while collaborating effectively with a multidisciplinary team
