What you'll be doing:

JIB Medical is seeking an experienced Dietitian to join our JIB Medical team of professionals. Reporting to the Chief Medical Officer, Medical Director, and Assistant Medical Director, the Dietitian will be responsible for but not limited to:

Clinical duties


• Provide one-on-one nutrition counseling to outpatient patients via in-person and telehealth

• Develop and implement an individualized care plan for nutrition intervention based on nutrition assessment and evidence-based standards

• Assess educational needs and provide nutrition counseling for individuals and groups

• Provide comprehensive, patient-centered education and support for managing diabetes, prediabetes, and cardiometabolic conditions

• Provide instruction on self-monitoring of blood glucose levels using various glucometers and CGM devices


• Collaborate with the interdisciplinary team regarding nutrition care plans

• Complete charting using EMR, the Nutrition Care Process and writing PES statements to provide appropriate documentation for the care plan in a timely manner

• Demonstrates accountability for the proper use of patients' protected health information

• Conduct nutrition and wellness workshops or group sessions

Sedentary work environment. Physical demands described are representative of those that must be met by an employee to successfully perform the functions of this job. This would require the ability to stand, bend, shred documents, open/close filing cabinets as necessary.

Position Type/Expected Hours of Work

This is a part-time position. Part time hours- a maximum of 18 hours a week. Preferably some Saturday hours. (We have flexibility with the hours-but would like some Saturday and some late-night hours)

Travel -- Occasional travel is required. Must be able and available to travel to distant locations, including Cutchogue, Long Island, Queens Long Island City Training Center, and other locations, as necessary.

Requirements

Required Experience, Skills, and Qualifications

• Ability to perform work independently with minimal training
• Excellent verbal and written communication skills
• Ability to communicate effectively with patients, family, and members of the health care team.
• Basic knowledge of application software such as Excel, Word, and PowerPoint.
• 3+ years of clinical nutrition experience
• Part-time, max 18 hours per week, (We have flexibility with the hours-but would like some Saturday and some late-night hours)
• Ability to use telehealth for virtual visits
• Current Registered Dietitian credentialed by CDR
• Certification by the State of New York as a Certified Dietitian Nutritionist (CDN)
• CDE/CDCES required
• Master's degree preferred
• Bachelor's degree with completion of required coursework and supervised practice as dictated by the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
