Social Services Director – Skilled Nursing & Rehabilitation

Gulfside Health & Rehab is seeking a compassionate, organized and resident-focused Social Services Director to join our interdisciplinary team.

We are a smaller 76-bed skilled nursing and rehabilitation facility , which provides an opportunity to work closely with residents, families and a tight-knit interdisciplinary team. We value a collaborative, family-oriented environment where department leaders are visible, accessible and actively involved in the day-to-day experience of our residents.

The ideal candidate will have a genuine desire to work with the geriatric population and an understanding that successful social services in long-term care requires more than completing required documentation. We are looking for someone who enjoys building relationships with residents and families, advocating for their needs and contributing positively to their quality of life.

Key Responsibilities

• Coordinate social services for residents in our skilled nursing and long-term care setting
• Complete psychosocial assessments and participate in interdisciplinary care planning
• Coordinate discharge planning and maintain an accurate discharge calendar
• Assist residents and families with adjustment to long-term care or short-term rehabilitation and connect them with appropriate resources
• Participate in MDS/RAI processes and required documentation
• Support resident rights, dignity, psychosocial well-being and quality of life
• Address resident and family concerns and assist with grievance resolution
• Collaborate closely with Nursing, Rehabilitation, Admissions, Activities, physicians and other members of the interdisciplinary team
• Maintain timely and accurate documentation in the electronic health record
• Participate actively in facility programs and initiatives supporting resident-centered care

Qualifications and Preferred Experience

• Previous healthcare social services experience preferred; skilled nursing or long-term care experience strongly preferred
• Previous experience as a Certified Nursing Assistant (CNA) is preferred and considered an asset
• Knowledge of MDS/RAI processes preferred
• PointClickCare experience required
• Strong communication, organization, problem-solving and interpersonal skills
• Ability to establish positive relationships with residents, families and staff
• Genuine interest in serving the geriatric and long-term-care population
• Bachelor’s or Master’s degree in Social Work or a related human-services field is preferred but not required for this position

Compensation:$56,000-$72,000 annually, based on relevant experience and education level.
