Social Work Care Coordinator- Behavioral Health Pavilion (PFK)

Nationwide Childrens Hospital

Overview Case Management Extender - BHP 1.0 FTE, 40 hours per week, Full-time benefits. Monday - Friday, 8am - 5pm. The Care Coordinator assesses key needs, addresses local and systemic issues interfering with care, and assists families in setting appropriate goals and tracking progress toward achieving those goals. The Care Coordinator navigates the health care system on behalf of high‑need patients, supervises and provides care coordination, plans for education, and provides for patient safety and quality of care. The incumbent will serve as an agile problem solver on multiple levels, practicing professionally, ethically, and promoting patient advocacy. Note: Regular, reliable and consistent attendance is an essential job function. Employees are expected to perform work as scheduled. Responsibilities Outreach to primary care physicians, specialty physicians and other service providers to identify patients in need of care coordination. Complete comprehensive Health Risk Assessment, screening, and identification of patients. Facilitate communication and collaboration among the healthcare team. Develop patients' and family self‑management skills through education and resource provision; develop family‑centered plan of care. Manage transitions between settings, caregivers, and providers. Provide monitoring and follow‑up across the continuum of care. Provide consultation, education, and training to staff regarding psychosocial issues that impact the delivery of optimal health care services. Participate in orientation and continuing education of staff and students as appropriate. Qualifications Education requirement: Master’s degree in social work or related field, required. Licensure requirement: LSW, LPC, required (LISW or LPCC preferred). Valid Ohio driver’s license and proof of auto insurance as required by hospital policy. Must pass motor vehicle background inspection, insurance eligibility, driving qualifications, and training set forth by Nationwide Children’s Hospital and maintain compliance with insurance guidelines. Certifications: Active BLS certificate; Case Management certification desired. Skills: Working knowledge of Medicaid and other regulatory agency standards. Experience: 2 years clinical or medical social work experience, required. Minimum physical requirements: Continuously (67‑100%) 5.1 to 12 hours or greater; Sitting, flexing/extending of neck, computer skills; hand use: grasping, gripping, turning; repetitive hand/arm usage; audible speech; hearing acuity; vision: near/distance; problem solving; decision making; interpreting data frequently (34‑66%) 2.6‑5 hours; walking; standing; driving; vision: depth perception, color changes, peripheral. Occasionally (1‑33%) 0.5‑2.5 hours: bend/twist/kneel/squat; climb stairs/ladders; reach above shoulders; lifting/carrying up to 20 lbs; push/pull up to 25 lbs; exposures to communicable diseases and/or pathogens; patient equipment; machinery; electricity.
