Health & Wellbeing Coach

The Health and Wellbeing Coach provides personalised support to individuals to empower them to take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of: Healthy Eating and Nutrition Smoking Cessation Physical Activity Weight Management Alcohol consumption NHS Health Checks (outreach) As a Health and Wellbeing Coach, you will facilitate people in establishing and attaining health‑promoting goals, set by the person to change lifestyle‑related behaviours, with the intent of reducing health risks, improving self‑management of chronic conditions, and improving quality of life. The Health and Wellbeing Coach is also responsible for delivering face‑to‑face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings via a combination of one‑to‑one and group‑based support in line with need, location and offer. To provide a health coaching service for eligible participants that supports people to gain and use the knowledge, skills, and confidence to become active participants in their health and wellbeing so that they can reach their self‑identified health and wellbeing goals, you will: Triage and assessment Agenda setting Lifestyle behaviour change support to (e.g.) stop smoking, move more, achieve a healthy weight/diet, reduce alcohol consumption levels. Problem‑solving Motivational interviewing Goal setting Signposting Delivery of multi‑channel, personalised health coaching support to individuals and groups in line with their needs and preferences includes: Face‑to‑face, telephonic, and digital health coaching Multi‑mix communication methods including in‑app messaging, text, and email. Sharing helpful and supportive resources in line with goal, stage of journey, confidence, and motivation. Advocating the use of behaviour tracking tools (online or offline) Appraising individuals and population caseloads daily to ensure participants are provided with the right intensity and frequency of support to drive sustained behaviour change. Using a strength‑based approach, maximising opportunities to utilise local community assets and efficient utilisation of the wider health and wellbeing economy. To deliver services in line with quality standards, national / government guidelines, and role boundaries, you must be able to identify when coaching is not appropriate and sensitively discuss alternatives or appropriately refer individuals to wider services. Support with outreach community engagement work includes, but is not limited to: Screening services (e.g., NHS Health Checks) Community and stakeholder events External meetings and networks Roadshows and pop‑ups Work in line with business and contract performance requirements includes: Implementing Standard Operating Procedures (SOP) Fulfilling policies and quality management framework expectations including data protection, information security, health and safety, and safeguarding. Participating in regular audits and performance observations, demonstrating a proactive commitment to providing high‑quality services and continuous professional development. Adopting a proactive approach to supervision and training with demonstrated commitment to personal development, achievement of quality standards, high performance in line with agreed objectives and KPIs. To ensure accurate reporting and data collection in line with contractual and case‑management system requirements you will: Manage caseloads efficiently, effectively and at scale. Support team members in population‑based behaviour change as required and in line with need. Collaborate with wider informal and formal carers/specialists to complement and enhance the behaviour change journey. Offer a comprehensive and coordinated supportive service so that participants only have to tell their story once. Ensure data audits accurately capture activity, productivity, utilisation, and outcomes. Qualifications & Experience Relevant health coaching qualification or an accredited health coaching skills programme. A minimum of six months of direct health coaching delivery. Experience delivering behaviour‑change interventions for specific lifestyle related issues (e.g., smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction). Evidence of supporting vulnerable individuals through a change process. Experience of caseload management demonstrated via use of a Case Management System. Experience of inputting and processing data with accuracy, meeting targets and deadlines in the workplace. Evidence of continuous professional development to ensure delivery of the most recent and up‑to‑date health coaching provision. Experience working in a health, community, or social care environment or strong understanding of the issues around maintaining a healthy lifestyle. Desirable Membership of professional bodies (ICF, EMCC, AoC, UKHCA). Experience managing and supervising individuals and teams. Degree level education in a relevant subject (e.g., Psychology, Behavioural Science). Experience supporting people remotely / telephonically / digitally. Evidence of working with individuals with long‑term health conditions. Individual Competencies A strong understanding of the social determinants of health. A strong understanding of population‑based approaches including segmentation and risk stratification. A solid grasp of behaviour‑change principles and methodology including the COM‑B model and behaviour change wheel. Excellent oral and written communication skills with people from a wide variety of backgrounds. Demonstrable core skills and competencies as set out in best‑practice standards including selecting and applying a range of health‑coaching models, conversation frames and techniques. Detailed knowledge of the core concepts and principles of personalisation, patient activation, health behaviour change, self‑efficacy, intrinsic motivation, and assets‑based approaches. In‑depth understanding of self‑management support and associated techniques. Advanced skills development, incorporating practice, reflection and planning for application in practice. Effective internal and external stakeholder engagement and management. Strong understanding of confidentiality and safeguarding principles. Strong written and verbal communication skills with the ability to capture essential information that supports effective case management. Proven track record of interpersonal relationships and ability to build, develop and maintain relationships. Effective caseload management including accurate data collection, timely recording, and delivery against agreed SLAs and SOPs. Confidence in using evolving digital technologies to support people through behaviour‑change processes. Ability to manage time independently and meet deadlines. Commitment to personal development and training. Commitment to flexible working (including evenings and weekends) to ensure service is available to all residents, including those who work during the day, shift workers and those with children of school age. Commitment to supporting overall team/contract performance to the highest standards, undertaking a range of duties in line with business requirements.
