Community Health Advocate/UKHC

Job Summary The Community Health Advocate provides client-centered outreach, education, and navigation services to individuals at risk for or living with HIV and Hepatitis C. Using a trauma-informed, culturally responsive approach, the role builds trusting relationships with clients and community partners to reduce stigma, increase awareness, and promote timely linkage and retention in care. The position assesses client needs, addresses barriers to care, and navigates healthcare and social service systems to connect individuals to medical, behavioral health, and supportive services. It ensures accurate documentation, data tracking, and compliance with confidentiality standards. Responsibilities Conduct community-based outreach and testing. Coordinate appointments and telehealth visits. Provide health education and risk-reduction counseling. Support treatment adherence in a defined service area. Maintain accurate and confidential client documentation. Collaborate with community partners to reduce stigma and increase awareness. Qualifications

• Required Education: BA
• Required Experience: 3 years (community outreach, case management, or related field)
• Required License/Registration/Certification: Driver’s License
• Preferred Education/Experience: Bachelor’s degree in Health Education, Social Work, Social Services, Nursing, or Human Development

Skills, Knowledge, and Abilities Proficiency in MS Windows applications (Word, Excel, Access or other database experience, Outlook) Data entry skills and database management. Health education and motivational interviewing techniques. Strong interpersonal and communication skills. Ability to work independently and as part of a team. Additional Information

• Position will require driving within a designated service area to provide linkage, navigation and outreach services.
• Shift includes evening and weekend work, and flexible hours outside of traditional office hours.
• This role is located in Louisville, KY and serves the Kentucky Area Developmental Districts of KIPDA in the Louisville area.
• No supervisory responsibilities.
