Job Summary The Case Manager assists with coordination of services through the continuum of care and in balancing quality patient care with cost‑effective utilization of resources. Key Responsibilities Coordinate admission, treatment, and discharge plans and ensure proper utilization of hospital services Ensure care is provided at the most appropriate level to meet patient care needs Serve as a liaison to ensure appropriate reimbursement for hospital stays and services Technical Capabilities Demonstrate mastery of nursing patient assessment and evaluation in complex clinical settings Provide patient education independently and without requiring additional support Integrate evidence from multiple sources to determine practice changes Demonstrate mastery of case management and treatment planning in complex clinical care coordination Deliver treatment planning services without requiring support or instruction Nursing Philosophy We believe highly skilled and specialized nursing care is essential to Vanderbilt University Medical Center’s mission of quality in patient care, education and research.
