Overview The Behavioral Health Social Worker MSW plays a pivotal role in carrying out the mission of the organization by providing high quality care including treatment planning, discharge planning, and utilization review to patients hospitalized on the mental health unit. This role assesses psycho-social needs, educates, counsels, and plans, coordinates, and implements care across the continuum while contributing to patient outcomes and length of stay. The Behavioral Health MSW works closely with other professionals (i.e., nursing, counseling staff, occupational therapy, psychiatry, psychology). It communicates teamwork, caring, and compassion with all internal and external healthcare team members. Responsibilities Complete and document a standard social work psycho-social assessment by addressing barriers to treatment, increasing patient/family satisfaction, improving appropriate utilization of resources, creating safety plans to address ethical and legal concerns, discharge and transitional care needs, trauma, financial problems, treatment compliance issues, substance use and other psychosocial barriers to maximizing health status. Provide social work intervention as it relates to hospitalization including crisis management, mental health and suicide resources, health care decision-making, illness, ethical/legal concerns, discharge planning, and transitional care needs, abuse, domestic violence, capacity, financial problems, compliance issues, as well as other psychosocial barriers to maximizing health status. Actively participate in care coordination efforts to identify high-risk factors and respond appropriately, including co-leading daily care coordination meetings with the care team. Work with the healthcare team, patient, family and/or significant others to continually transition the patient to the appropriate level/place of care and ensure avoidable days are captured and documented. Advocate, mediate, and negotiate with an emphasis on self-determination for the patient system. Formulate a cohesive plan for maintaining the patient’s health status, improving social supports, and moving the patient safely into less restrictive, less costly levels of care according to available resources. Meet the comprehensive needs of the patient in their environment to avoid readmission. Integrate relevant theories of family dynamics, crisis intervention, strengths-based, solution-focused approaches, and meaning of illness into every work with patients. Participate in care coordination efforts to identify high-risk factors and respond appropriately so that key information is captured and documented. Participates in daily team meetings. Collaborates with outside agencies, sets follow-up appointments, and works with CMH for additional resource assistance with placement and discharge plan. Builds professional relationships with patients, families, and the treatment team to address conflict resolution with positive outcomes. Communicates teamwork, caring, and compassion. Provides excellent customer service in accordance with the mission, vision, and values of the health system. Assumes responsibility for continuing professional development and maintains licensure. Provides for the age-specific needs of the population served according to evidence-based standards and organizational policies/procedures for the adult and older adult populations. Other duties as assigned. Membership in at least one professional organization (i.e., NASW, SSWLHC-MI Chapter, national SSWLHC, etc.) preferred. Certifications and Licenses MSW: Masters of Social Work Licensure as a Master’s Social Worker in the State of Michigan (or actively working towards). Limited License MSW employees must be actively working to obtain full licensure. These employees must pass licensing exam within two years of eligibility and work 4,000 hours supervised by LMSW. ICHAT: iChat (annually on Hire Date) TEAM ADVANCED: Techniques for Effective Aggression Management Within 6 months of hire or transfer (Employees who are in this position on or before 12/31/2023 will have 24 months to complete this certification) FINGERPRIN: Fingerprinting Required Education Master’s degree is required Other Information REQUIRED EXPERIENCE, TRAINING AND SKILLS: Mandatory Recipient Rights training within 30 days of hire and annually is required. Mandatory nonviolent crisis intervention and physical management training (CPI) within 90 days of hire and annually thereafter. Training and experience, and/or demonstrated competency in casework principles, including diagnosis, assessment, crisis intervention, treatment and transition planning; techniques in motivational interviewing and short-term, solution-focused therapy; awareness of community resources, public assistance and entitlement programs, ensuring transition to appropriate levels of care; and issues relating to age and stage of development, special needs and cultural patterns of patient population served. Knowledge of social work practices and processes related to the care of behavioral health patients. Interpersonal skills needed to communicate successfully with individuals and groups and interact with people at all levels to communicate ideas and concepts in a clear and understandable manner. Must adhere to the National Association of Social Workers’ Code of Ethics. Must follow MyMichigan Health and The Michigan Mental Health Code policies and mandatory educational competency requirements. PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS: Exposure to stressful situations, including those involving public contact, as well as trauma, grief and death. Able to wear personal protective equipment that includes latex materials or appropriate substitute if required for your position. Able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Vision and hearing are necessary with or without assisted devices. Frequently required to sit/stand/walk for long periods. May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material. Must follow MyMichigan Health bloodborne pathogen and TB testing as required. Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance, and stay awake. Overall dexterity is required including handling, reaching, grasping, fingering and feeling. May require repetition of these movements on a regular to frequent basis. Physical Demand Level: Light.
