DESCRIPTION: This is paraprofessional support work in training, providing direct care and support, and implementing rehabilitative program for mentally ill or developmentally disabled consumers. Work includes supervising lower level Mental Health Workers, counseling consumers and families, ensuring client daily living needs are met, administering prescribed medication, maintaining physical environment, charting and maintaining consumer progress records, and participating in treatment and rehabilitative programming. Responsibilities may include being in charge of a ward or specific patient activity area or a member of an intensive case team. Work is performed under general supervision. REPRESENTATIVE TASKS: (A position may not be assigned all the duties listed, nor do the listed examples include all the duties that may be assigned). Trains clients in order to assist them in adjusting to the work environment and living situation. Oversees a residential unit, ward, or patient activity area in order to ensure resident needs are met. Assigns, schedules, directs, and oversees the work of Mental Health Workers in order to ensure compliance with prescribed programs and services. Administers and records prescribed medications in order to carry out orders of medical staff and maintain required documentation. Evaluates, orders, coordinates, and distributes necessary supplies and equipment in order to ensure resident environmental living standards are maintained at appropriate levels. Coordinates client programming with referral source in order to maintain appropriate case plans. Implements and monitors individual client case plans in order to help client achieve stated goals and objectives. Counsels individuals in order to resolve personal problems and provide understanding of acceptable personal and social behavior. Checks on residents in order to ensure appropriate care and treatment in the areas of hygiene, food, clothes, medicine, and therapy are provided. Checks wards and environment in order to ensure cleanliness and safety. Reviews and comments on written reports from subordinate Mental Health Workers in order to ensure accurate resident information is provided. Studies client psycho social behavior in order to identify problems and locate appropriate services. Manages cases and serves as group leader for clients in order to provide treatment and assist them in reaching established goals. Assesses needs, orders and shops for clients’ clothing, personal items, money, and equipment in order to ensure client’s personal needs are met. Describes services provided to clients to other staff in order to maintain consistency in care and ensure patient rights. Trains lower level Mental Health Workers in order to maintain competent staff. Plans, schedules, and coordinates community events, placement visits, and recreational and other activities for residents in order to coordinate off grounds activities. Records medications, behavior, and other facts on patient charts in order to maintain patient records. Confers with families and community service providers prior to, during, and subsequent to community placement in order to act as liaison and coordinate services. LICENSING/REGISTRATION/CERTIFICATION REQUIREMENTS: (These must be met by all employees prior to attaining permanent status in this class). Some positions may require certification to administer medication, CNA, or completion of a Department approved training program.
