Spiritual Care Coordinator: Home Health and Hospice Concordia Hospice of Washington & Concordia Visiting Nurses At Concordia, faith is at the heart of everything we do. We are seeking a part-time Spiritual Care Coordinator who feels called to serve others through faith, compassion, and whole-person care. Whether supporting patients receiving hospice services or home health care, you will provide spiritual guidance, emotional support, and a comforting presence while honoring each individual's beliefs, values, and traditions. Meaningful work that makes a lasting difference in the lives of patients and families A Christian, mission-driven organization dedicated to serving others with compassion and dignity Supportive interdisciplinary team including nurses, social workers, therapists, aides, and volunteers Independence in the field while remaining connected to a caring and collaborative team The opportunity to use your gifts to bring hope, comfort, and peace during life's most vulnerable moments What You'll Do Provide spiritual care, prayer, counseling, and emotional support to patients, families, and caregivers Assess spiritual needs and incorporate them into each patient's plan of care Offer comfort through presence, conversation, scripture, prayer, or other meaningful spiritual practices while respecting all faith traditions and belief systems Support patients and families through illness, grief, end-of-life care, and times of transition Collaborate with nurses, social workers, aides, therapists, physicians, and other members of the interdisciplinary care team Participate in care planning meetings and document spiritual care visits Lead or assist with memorial services, bedside prayers, and family support as appropriate Who You Are You lead with compassion, empathy, and grace. You understand that every patient's spiritual journey is unique and are committed to providing individualized support that honors their beliefs while reflecting Concordia's Christian mission.
