The LPN collects and analyzes data from the patient's medical history, nursing assessments and staff input on an ongoing basis. The LPN provides PRN visits on patients who call the answering service, crisis care visits as assigned and PRN visits as requested by PCC or MD. Provides care for patients for the entire County. The LPN works collaboratively with the interdisciplinary care team to ensure high-quality, patient-centered care that promotes comfort, dignity, and quality of life.Current and active Florida (LPN) License required.
