Case Management Assistant: Drive Discharge & Utilization

Trinity Health

Trinity Health in Fresno, California, is seeking a full-time Care Coordinator support to assist with utilization review management and discharge planning for acute care patients.

Requirements include a high school diploma, two years of acute care experience, strong communication, medical terminology competency, computer data entry, and a current CPR card. Prior case management or utilization review experience is preferred.
