Bilingual In-Home Case Manager: Family Support Advocate
Catholic Charities Of The Diocese Of Oakland
Catholic Charities Of The Diocese Of Oakland is seeking a Path 2 Case Manager to provide in‑home case management for the DR program in Contra Costa County. You will assess families, develop action plans, and coordinate referrals while maintaining accurate documentation.
