This is an entry level clinical pharmacist position. The clinical pharmacist supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills. EEO/AA/Disability/VeteranResponsibilitiesCustomer Service Quality Demonstrate and promote CHAMP behaviorsProvide excellent customer service to patients and other healthcare team members, with attention to the needs of all staff, with positive feedback from staff and co-workersProvide medication counseling to patients and families, as applicable to work areaDemonstrate effective oral and written communication skillsDemonstrate ability to escalate issues to management and/or collaborating prescribers as appropriateMedication Management Conduct timely, accurate medication order review that considers all patient characteristics including laboratory values and vital signsProvide medication therapy monitoring for assigned patients, adjusting medication therapy based on patient factorsDevelop an effective interdisciplinary relationship with the health care teamAct as a source of medication information for the health care teamParticipate in patient-centered initiativesEnsure clinical continuity where applicable to work areaMedication Preparation and Distribution Demonstrate competency in pharmacy automation and technology as applicable to work areaOversee sterile and non-sterile compounding to ensure compliance with regulatory statutes (e.g. USP Chapters), as applicable to work areaOversee medication distribution/dispensing process in work area, to ensure safe, effective, timely medication distribution/dispensingDemonstrate competency in and oversee controlled substance management, including monitoring and documentationMedication Safety, Regulatory Affairs, and Quality Assurance Adhere to all departmental policies, procedures, protocols, and guidelinesAdhere to appropriate state and federal regulationsReport medication events and near missesDemonstrate HRO principles and conduct in all interactions with patients and staffParticipate in quality assurance activitiesLeadership Provide education and training to other pharmacy staff members, pharmacy students and residents, and health care team members.Participate in professional development activitiesAttend departmental staff meetingsOversee pharmacy staff as necessary in work areaFinancial/Volume Results Contribute to departmental cost-reduction/revenue generationParticipate in pharmacist-driven cost management protocols where applicableProvide cost-effective clinical careThe pharmacist has successfully completed an immunization training program that complies with the requirements of section 20-633-3 and section 20-633-4 of the Regulations of Connecticut State AgenciesQualificationsEDUCATION(number of years and type required to perform the position duties): Graduation from an accredited College of Pharmacy with a B.S. or PharmD degreeEXPERIENCECompletion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience is desirable, but not required.LICENSURECT Pharmacist Licensure required, active and without restrictions. If required, will be responsible for obtaining out of state licenses to register for non-resident pharmacy licenses. BLS (Basic Life Support) certified or obtained within 6 months of hire. Immunization certified or obtained within 6 months of hire.SPECIAL SKILLSThis individual should demonstrate basic knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Excellent verbal and written communication skills and the ability to work with others are required.
