Hospice Registered Nurse Case Manager

Hospice of Missoula is seeking a Registered Nurse case manager to oversee care delivery for patient/family caseloads and supervise CNAs, home makers, and massage therapists. The role requires performing intake assessments, developing plans of care, making referrals, coordinating with the hospice team and ensuring proper documentation.
