About the Position

• Coordinate and manage patient care throughout the hospital stay.
• Develop, implement, and monitor individualized case management plans.
• Collaborate with physicians, patients, families, nurses, social workers, and interdisciplinary teams.
• Evaluate patient needs and coordinate appropriate levels of care and healthcare resources.
• Support utilization management and ensure cost-effective delivery of patient care.
• Coordinate discharge planning and transitions to appropriate post-acute care services.
• Communicate with insurance companies, payers, and review organizations regarding patient care and utilization.
• Maintain clear, accurate, and timely case management documentation.
• Educate physicians and staff regarding utilization, level-of-care, and discharge planning issues.
• Identify barriers to discharge and coordinate appropriate solutions with the care team.
• Maintain compliance with applicable federal, state, payer, and hospital regulations.
• Participate in departmental meetings, quality improvement initiatives, and continuing education.

Requirements

• Bachelor’s Degree in Nursing (BSN) required; Master’s degree preferred.
• Minimum 3 years of clinical experience in a hospital setting.
• Minimum 5 years of hospital-based case management experience.
• Current unrestricted Registered Nurse (RN) license in the State of Colorado required.
• Current BLS certification required.
• Case Management certification preferred or willingness to obtain certification within 2 years.
• Strong communication, documentation, care coordination, and organizational skills.
• Knowledge of utilization management, discharge planning, and the continuum of care.

Weekend Coverage: 5 hours for one Saturday each month; take 5 hours off on one weekday
