Legacy House of Southern Hills is on the lookout for a dedicated and experienced full-time Memory Care Program Coordinator to take the lead in our Memory Care unit.Here's what we offer to support your well-being and success:$50,000 per year, depending on experience401(k) with company matchComprehensive health, dental, vision, and life insuranceHealth savings account (HSA)Paid time off (PTO) and vacationGenerous employee referral programHotel discounts at company-managed locationsWhat will you do as our Memory Care Program Coordinator?Care Assessment & Planning: Conduct initial and ongoing evaluations of residents' cognitive and physical abilities and develop individualized care plans in collaboration with the care team.Team Leadership: Supervise care staff to ensure high-quality support is provided and train team members on effective memory care techniques and communication methods.Resident Engagement: Lead daily activities that promote cognitive health and social connection, including individual and group programs tailored to memory care needs.Family Support: Act as the primary contact for families, offering regular updates and providing resources to help them navigate memory-related challenges.Documentation & Compliance: Maintain detailed and accurate records of resident progress and care and participate in meetings to review and adjust care strategies.Healthcare Collaboration: Partner with medical professionals to coordinate integrated care and ensure services meet regulatory standards and memory care best practices.Environmental Oversight: Monitor and adjust the memory care environment to ensure it is safe, secure, and supportive of residents' well-being and independence.When will you work?This full-time position offers a consistent Tuesday through Saturday schedule.What do you need to be our Memory Care Program Coordinator?Medication Technician certificationExperience in healthcare or social services, particularly in gerontology or memory careKnowledge of memory disorders and understanding of therapeutic interventionsAbility to manage a team and promote a positive, supportive environmentStrong communication and interpersonal skillsWondering who we are?At Legacy House of Southern Hills, we're committed to helping our senior residents live as independently as possible-because independence fosters health, happiness, and dignity.
