Description

American Traveler is seeking an RN Care Manager for a Home Health position requiring a CA RN license and EPIC experience.

Details

• Home health setting providing skilled nursing care to patients in their homes
• Manages an assigned patient caseload independently while collaborating with a multidisciplinary care team
• Day shift schedule from 8:00 AM to 4:30 PM
• EPIC EMR experience required

Requirements

• Active RN license in California required
• Current BLS/CPR certification from an approved vendor required
• Valid driver's license required, must be 21 years or older
• Current automobile insurance and reliable personal vehicle required
• BSN preferred
• Prior acute care facility or home care experience preferred
• Minimum of 2 years of experience in the specialty required

Additional Information

• Completes initial and ongoing comprehensive patient assessments and updates plans of care as needed
• Provides direct and indirect skilled nursing care and coordinates services with other disciplines
• Educates and counsels patients and families on healthcare and nursing needs
• Documents patient care delivery, status, and response to treatment in accordance with policy
• Accepts local candidates at the same rate as travel candidates with no radius restriction
• Candidates with more than 5 days of requested time off will not be considered
• Candidates with any prior adverse licensure action will not be considered
