Heartland Hospice seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will deliver direct care during nights and weekends, coordinate plan of care, and collaborate with the IDG to ensure seamless patient care. This role requires an active RN license, CPR certification, and at least one year of relevant hospice or home health experience.
