RN Case Manager

Join the dedicated Case Management team at Grand Strand Medical Center , where you will play a critical role in ensuring patients receive the right care at the right time while helping navigate complex healthcare needs throughout their hospital stay. Key Responsibilities Assess patient needs and develop individualized discharge and transition plans. Coordinate patient care across the continuum, including post-acute services and community resources. Collaborate with physicians and clinical staff to identify barriers to discharge and develop solutions. Perform utilization review activities and monitor appropriate resource utilization. Educate patients and families regarding treatment plans, discharge instructions, and available support services. Facilitate interdisciplinary care team meetings and communicate care progression. Ensure documentation meets regulatory, payer, and organizational requirements. Advocate for patients to ensure safe, effective, and timely care delivery. Maintain knowledge of healthcare regulations, reimbursement guidelines, and case management best practices. What qualifications you will need: Active and unrestricted Registered Nurse (RN) license. Graduate of an accredited nursing program. Current BLS certification. Minimum of 2 years of clinical nursing experience in the acute care setting required. Prior case management experience required.
