Experienced Social Worker (SW) – Vineland, NJ
An established healthcare organization in Vineland, NJ is seeking a dedicated and compassionate Social Worker to join its multidisciplinary team. This role plays a vital part in enhancing residents’ quality of life through supportive care and thoughtful coordination.
Key Responsibilities:
Support residents in maintaining autonomy and addressing psychosocial needs
Assess individual emotional and social needs and recommend appropriate interventions
Develop and implement personalized care plans
Collaborate on discharge planning, community reintegration, and transfers
Participate in care plan meetings to ensure holistic, resident-centered treatment
Qualifications:
Bachelor’s Degree in Social Work, Psychology, or Human Services
Active New Jersey Social Work certification or license
Prior experience in long-term care or subacute settings required
