For submission prior to hiring, a resume and two professional references are required. Optional items include positive PPD-negative chest X-ray records, name verification affidavit, badge photo, and prime COA. Additionally, license/certificate verification is required. No specific educational degrees, clinical or leadership experience criteria, or skills are explicitly detailed beyond these documents and health checks.
