Occupational Therapist – Home Health

Portland, OR | Home Health

Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:00 AM – 5:00 PM | Onsite

Key Responsibilities

• Provide occupational therapy evaluations and treatments to patients within a home health setting
• Manage a caseload within a 50-mile radius of the office
• Meet productivity expectations of 30 points per week (6 points per day)
• Document patient care accurately and in a timely manner using HCHB (Homecare Homebase)
• Develop and implement individualized treatment plans in accordance with physician orders and patient goals

Required Qualifications

• Active Oregon (OR) Occupational Therapist license required prior to start
• Active Washington (WA) Occupational Therapist license required prior to start
• Previous home health experience required
• Previous HCHB (Homecare Homebase) experience required

Preferred Experience / Skills

• Familiarity with home health productivity standards and point-based systems
• Strong time management and ability to work independently in a field-based environment
