Clinical Social Worker

The Integrated Care Center Clinic is a federally qualified health center that provides primary care, behavioral health, addiction treatment, psychiatry, dental care, and more. We provide care to those most in need including those who are suffering from homelessness, mental illness, substance use, mental illness, and chronic medical conditions. We provide team based care in an integrated care setting. This is a union-eligible position.

  1. The Mobile and Street Medicine program delivers comprehensive, high-quality medical care and social services to individuals experiencing homelessness in San Francisco. Rooted in the mission to "go to the people", the program offers services directly to patients on the street, in encampments, or via a fully equipped mobile medical van. The program operates as an extension of our Integrated Care Center (ICC), expanding the reach of our services beyond the clinic walls. This new and rapidly growing initiative is committed to providing low-barrier, trauma-informed, and person-centered care in the environments where people live. Our multidisciplinary team prioritizes trust, continuity, and access while delivering compassionate, dignified, and high-quality services to some of the city's most underserved populations.
  2. The Clinical Social Worker provides behavioral health therapy and integrated case management to individuals experiencing unsheltered homelessness and complex medical, psychiatric, and social needs. This role combines field-based case management with direct clinical mental health services and plays a key role in strengthening and expanding the Mobile & Street Medicine program's behavioral health capacity. This position is highly collaborative, flexible, and hands-on. The ideal candidate thrives in dynamic field environments, enjoys wearing many hats, and brings creativity, leadership potential, and a commitment to building new systems of care alongside a multidisciplinary team. This is a full-time, in-person, field-based position. This position may be filled by an ASW practicing under clinical supervision, which will be provided by an LCSW employed by HR360, or by an independently licensed LCSW.

    KEY RESPONSIBILITIES

    • Provide mental health assessments, diagnosis, and ongoing psychotherapy to individuals with serious mental illness, trauma histories, substance use disorders, and co-occurring conditions.
    • Effectively de-escalates clients experiencing psychiatric or emotional crises and facilitates appropriate interventions to ensure safety and continuity of care.
    • Utilize evidence-based, trauma-informed modalities appropriate for street-based care (e.g., harm reduction, motivational interviewing, CBT-informed approaches).
    • Collaborate closely with psychiatry, addiction medicine, primary care providers, and nursing staff.
    • Participate in interdisciplinary case conferences and behavioral health consultations.
    • Engages collateral contacts and utilizes appropriate referral sources, including hospitals, inpatient units, community providers, and support systems, to support comprehensive client care.
    • Contribute to the development of workflows, protocols, and program design to expand the team's behavioral health capacity.
    • Provide Enhanced Care Management (ECM) services in alignment with Medi Cal Managed Care Plan requirements, including comprehensive biopsychosocial assessments and individualized care planning.
    • Provide intensive, non ECM case management services for individuals with complex medical, behavioral health, and social service needs who require frequent coordination and support but do not meet ECM eligibility criteria.
    • Maintain an active caseload of individuals experiencing homelessness with complex medical, behavioral health, and social needs.
    • Develop, implement, and regularly update individualized, client-centered care plans in collaboration with patients and the multidisciplinary Mobile & Street Medicine team.
    • Deliver field-based services in locations accessible to clients, including streets, encampments, shelters, hospitals, residential programs, and the mobile medical van.
    • Coordinate care across healthcare, behavioral health, housing, substance use, legal, and social service systems.
    • Support care transitions, including hospital discharges, inpatient psychiatric admissions, and movement between housing or care settings.
    • Serve as a strong client advocate, navigating complex systems and addressing barriers to care.

    Program Development and Future Leadership

    • Actively contribute to the growth and evolution of case management and behavioral health services within the Mobile & Street Medicine program.
    • Provide informal mentorship, coaching, and clinical support to ECM Case Managers and behavioral health team members.
    • Assist with training, onboarding, and workflow development related to case management and mental health services.

    Documentation and Administration Responsibilities

    • Maintain timely, accurate, and comprehensive documentation in the electronic medical record, including ECM requirements.
    • Ensure documentation supports quality care delivery, continuity of care, and billing compliance.
    • Complete releases of information, assessments, care plans, progress notes, and required reports.
    • Lead weekly case discussions in collaboration with other clinical staff and program leadership.
    • Participate in required meetings, trainings, quality improvement initiatives, and supervision.

    Field Based Responsibilities

    • Travel frequently to provide services in community-based and non-traditional settings.
    • Work in varied environments, including outdoors and in challenging conditions, while maintaining professionalism and safety.
    • Maintain flexibility in daily workflow based on patient needs, outreach priorities, and team schedules.

    Organizational Responsibilities

    • Participates in quality improvement projects for the Mobile & Street Medicine program.
    • Maintains availability for various administrative and clinical duties as requested by the supervisor.
    • Represents HealthRIGHT 360 at and/or attends professional conferences, in-service training, and meetings at the request or with approval of supervisor
    • Complete all mandatory training courses on an annual basis.
    • May oversee students and other learners as appropriate.

    Performance and Quality Improvement Responsibilities

    • Meets established benchmarks for applicable visit volume, caseload expectations, and quality indicators, as assigned.
    • Meets benchmarks and performance measures for all contract- and grant-related deliverables.
    • Demonstrates strong initiative, enthusiasm, and willingness to contribute beyond assigned duties as needed to support team and program needs.
    • Actively participates in continuous quality improvement efforts through ongoing identification of problems and opportunities for improvement.
    • Assists with problem-solving and implementation of improvement strategies and, when requested, serves as a change champion or lead for quality improvement activities.
    • Engage in ongoing clinical supervision and professional development, including required supervision by an LCSW at HR360 for ASWs.

    QUALIFICATIONS:

    Education, Credentials, and Experience

    • Master's degree in social work (MSW) from an accredited program.
    • Registered Associate Clinical Social Worker (ASW) in good standing with the California Board of Behavioral Sciences, eligible and willing to practice under clinical supervision or Licensed Clinical Social Worker (LCSW).
    • At least 1 year of clinical mental health therapy experience, including assessment, diagnosis, and treatment required.
    • Case management experience with high-acuity populations.
    • BLS/CPR certification.
    • Valid California Driver's License.
    • Access to registered and insured transportation, and a clean driving record.
    • Must meet HealthRIGHT 360's motor vehicle record (MVR) standards.

    Desired

    • Enhanced Care Management (ECM) experience, or other intensive case management equivalent.
    • Ability to provide services in Spanish and English.
    • Experience in inpatient or hospital-based social work.
    • Leadership experience or demonstrated leadership potential.
    • Experience in homeless healthcare and Street Medicine strongly preferred.
    • Experience working with co-occurring mental health and substance use disorders.
    • Must complete background check and Livescan.
