RN Case Manager

Role RN Case Manager Responsibilities Conduct patient assessments, develop and implement care plans, coordinate services, facilitate communication among healthcare teams, monitor patient progress, manage transitions of care, educate patients and families, maintain accurate records, and participate in quality improvement initiatives to enhance patient outcomes and resource utilization. Qualifications Bachelor's Degree in Nursing required; Master's preferred. Minimum of 2 years clinical nursing experience, preferably in acute care. Current Illinois RN license required; CCM certification preferred. Skills Strong assessment, communication, and coordination skills, knowledge of healthcare regulations, and proficiency in electronic health records.
