Physical Therapy Assistant

Physical Therapy Assistant for a 13-week travel assignment in a skilled nursing facility south of San Jose. Responsible for implementing PT treatment plans, supporting patient mobility and independence, and maintaining documentation in an electronic medical record. Competitive weekly compensation up to $2,100 based on experience.

Key Responsibilities:

• Implement physical therapy treatment plans developed by the Physical Therapist
• Assist patients in improving mobility, strength, balance, endurance, and functional independence
• Provide therapeutic exercises, gait training, transfer training, and mobility interventions
• Monitor patient progress and accurately document treatment outcomes
• Educate patients and families on exercise programs, mobility techniques, and injury prevention
• Collaborate with physicians, physical therapists, nursing staff, and other healthcare professionals
• Maintain compliance with facility policies, state regulations, and professional practice standards
• Support patients in achieving maximum functional mobility and independence
• Maintain accurate and timely documentation within the electronic medical record

Qualifications:

• Associate Degree from an accredited Physical Therapist Assistant program
• Current California Physical Therapist Assistant license
• Current BLS certification
• Strong communication, documentation, and patient care skills
• Ability to work collaboratively within an interdisciplinary rehabilitation team

Preferred Skills:

• Previous Skilled Nursing Facility experience
