Implement and deliver physical therapy services across multiple locations to help patients improve mobility and recover from injuries or illnesses. Ensure compliance with treatment plans and support patient-centered care through effective therapy programs. Responsibilities Assess patients' physical conditions and develop individualized treatment plans. Administer therapeutic exercises and modalities to promote healing. Collaborate with healthcare teams to coordinate patient care across different sites. Document patient progress and update treatment plans accordingly. Educate patients and their families on exercises and injury prevention strategies. Adapt therapy approaches based on patient responses and clinical evaluations. Qualifications Hold a valid state licensure as a Licensed Physical Therapist. Demonstrate experience in outpatient or inpatient physical therapy settings. Possess strong clinical assessment and communication skills. Be adaptable to work across multiple clinic locations. Maintain current CPR and BLS certifications.
