How you’ll make an impact in this role

• Execute personalized care plans for renal and diabetic patients, monitoring clinical status to ensure rapid interventions and optimal recovery outcomes.
• Orchestrate multidisciplinary care teams as the primary clinical coordinator, ensuring a seamless and holistic treatment experience for every patient.
• Translate complex physician and nursing orders into precise clinical actions, maintaining the highest standards of safety and care integrity.
• Engineer comprehensive discharge strategies in collaboration with the healthcare team to empower patients for success and reduce readmission rates after they leave our care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
•  Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
