Job Title: Registered Nurse - Home Health

Location : Illinois

Salary: $80,000 - $85,000 annually

Responsibilities

• Provide skilled nursing care during scheduled home visits.
• Perform comprehensive patient assessments and ongoing evaluations.
• Complete OASIS assessments and ensure timely, accurate clinical documentation.
• Develop, implement, and update individualized plans of care.
• Administer medications, treatments, wound care, and other skilled nursing services as ordered.
• Monitor patient progress and communicate changes in condition to the physician and care team.
• Educate patients and caregivers on medications, disease management, and home safety.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Ensure compliance with Medicare, CMS regulations, and agency policies.
• Maintain accurate documentation using the agency's electronic medical record system.

Qualifications

• Active Illinois Registered Nurse (RN) license.
• Minimum of 3 years of home health nursing experience preferred.
• Strong experience completing OASIS-E1 assessments.
• Knowledge of Medicare home health regulations and documentation requirements.
• Experience with Homecare Homebase (HCHB) is preferred.
• Excellent clinical assessment, communication, and organizational skills.
• Current BLS certification.
• Valid driver's license, reliable transportation, and willingness to travel throughout the Chicagoland area.
