Position Summary We're seeking a highly proactive, compassionate, and relationship-driven Care Manager to serve as a vital connection between clients, families, caregivers, and the administrative team. In this dynamic role, you will lead prospective client follow-up and new client intake efforts while ensuring exceptional continuity and quality of care throughout the client experience. The ideal candidate combines clinical knowledge with strong interpersonal and organizational skills, excelling at building trust with families, guiding clients through the onboarding process, and supporting caregivers in the field. You will play a central role in converting inquiries into active clients, conducting in-home assessments and intakes, developing and updating Plans of Care, and collaborating closely with scheduling and care coordination teams to ensure services are delivered with compassion, consistency, and excellence. Key Responsibilities Directly report to and work together with the General Manager Harrison Skolnik and David Legare, RN. Primary follow‑up of prospective clients, answer questions, explain services, make connections with prospects/families, schedule intakes, and document in WellSky. Conduct new client intakes, explain services in detail, obtain signed service agreements and Plans of Care. Enter intake, care summary, and care log notes in WellSky; set a task to notify all office personnel of new client details. Call caregivers to introduce yourself and role(s), especially when the caregiver is new to a case; answer questions and establish rapport. Complete on‑site supervisory visits of caregivers and clients on a routine basis; ensure services are appropriate and consistent with POC; complete new POC as needed and communicate with Client Care Coordinators. Provide on‑site training as needed, counseling, coaching and educating caregivers and clients; make requests for follow‑up from other staff when outside scope. Collaborate closely with Client Care Coordinators on scheduling recommendations. Contribute to performance evaluations of caregivers/home health aides. Identify marketing opportunities with current clients; provide coordination letters and follow‑up visits; document visits and set follow‑up tasks with referral sources. Update and maintain WellSky database with daily schedule, client contacts, referral sources, activity logs and follow‑up plans. Maintain client confidentiality in accordance with HIPAA. Adhere to policies and procedures in compliance with AHCA, CDC, and OSHA guidelines. Complete other business‑related tasks as identified or assigned by Agency Administration. Required Qualifications Registered Nurse with 2+ years of experience. Administrative healthcare experience of 2+ years. Home health care supervisory experience of 2+ years.
