Responsibilities
- Administer medications and treatments according to physician orders and facility policy.
- Monitor residents’ health status, document changes, and escalate concerns to the supervising RN.
- Assist with activities of daily living and provide basic nursing care as needed.
- Maintain accurate and timely documentation in resident records (EMR/MAR) and update care plans.
- Respond promptly to call lights, ensuring resident safety and comfort.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.

Requirements
- Active Licensed Practical Nurse (LPN) license.
- Ability to work across all shifts; 40 hours guaranteed per week.
- Experience in long-term care or geriatric nursing is preferred.
- Strong documentation and communication skills; ability to work in a fast-paced setting.
