Field Care Manager RN - In-Home Wellness & Care
Humana
Humana is seeking a Field Care Manager Nurse (Physical Health) to assess and coordinate care for Medicaid members in Louisiana. You will partner with members and families to identify needs, address barriers, and connect them with clinical, community, and social resources. This role requires an active Louisiana RN license, 2+ years in-home care management, and the ability to travel up to 50 miles.
