Responsibilities:

• Analyze high-cost medication orders and identify opportunities for cost savings through therapeutic interchange while ensuring patient safety 
• Conduct comprehensive medication reviews to assess appropriate drug selection, dosage, administration, potential drug interactions, and opportunities for cost-effective therapy. 
• Recommend appropriate therapy changes and therapeutic interchange options to physicians and facility staff. 
• Evaluate medication orders for appropriateness of drug, dosage, potential interactions and proper administration. 
• Communicate effectively via, phone, and email. 
• Utilize Microsoft Excel to run hourly reports.  
• Complete any other task assigned by the Cost Containment supervisor. 

Qualifications:

• Licensed Pharmacist in the State of Florida 
• State license are plus PA, AL,OH, GA,NY 
• Long Term Care or Hospital experience preferred 
• Strong clinical skills, critical thinking, and analytical skills. 
• Excellent written and verbal communication skills 
• Strong attention to detail and organizational skill 
• Framework / ECM software knowledge a plus 
• Microsoft Excel knowledge skills 

Shift :

• 8:00am - 4:30pm with rotating weekends and holidays.
• Onsite
