Description

• Conduct assessments for patients in both inpatient and outpatient settings, crafting personalized treatment plans that cater to their unique needs
• Implement physical therapy interventions with a focus on enhancing mobility, strength, balance, and functional independence
• Work closely alongside nurses, physicians, case managers, and allied health professionals to enhance patient outcomes
• Provide education to patients and their families regarding therapeutic objectives, discharge strategies, and home exercise regimens
• Maintain thorough documentation of assessments, treatments, and patient progress in line with regulations and facility guidelines
• Engage in interdisciplinary rounds and team conferences to ensure seamless care coordination

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from a recognized program

Certifications

• Active state licensure as a Doctor of Physical Therapy
• CPR certification recommended
• Specialty training (e.g., in neurology or geriatrics) is advantageous but not mandatory

• Previous clinical experience is highly favored
