The LPN provides high-quality nursing care to residents while working closely with Registered Nurses, physicians, and the interdisciplinary team to promote each resident's health, comfort, and well-being in our long-term care and rehabilitation facility.QualificationsCurrent Licensed Practical Nurse (LPN) licenseCurrent BLS/CPR certification requiredStrong clinical and assessment skillsExperience in long-term care or rehabilitation preferred, but not requiredExcellent communication and interpersonal skillsCompassionate, dependable, and resident-focusedAbility to work effectively in a team environmentKey ResponsibilitiesProvide direct nursing care to residents in accordance with individualized care plans.Administer medications and treatments as prescribed.Monitor residents' conditions and promptly report changes to the RN or physician.Accurately document assessments, treatments, and resident responses.Assist with admissions, discharges, and physician orders.Collaborate with the interdisciplinary team to provide exceptional resident care.Supervise Certified Nursing Assistants (CNAs) and support quality resident care.Maintain compliance with all state, federal, and facility policies and regulations.Promote a safe, clean, and compassionate environment for residents and families.If you're a nurse who loves long-term care, values strong benefits and growth opportunities, and prefers a close-knit environment.
