JOB SUMMARY

Under the supervision of the Clinic Manager, the CNA is responsible for providing and assisting participants with activities of daily living and other assigned duties within the facility. Monitors health status and provides a safe environment for participants. Functions as a member of interdisciplinary team. Promotes professional working relationships with both internal and external customers.  Adheres to and supports all organizational policies and procedures and standards. 

SHIFT/SCHEDULE: 7:00 A.M - 3:30 P.M

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
• Follows facilityschedulefor daily/weekly assignments. 
• Participates inparticipant’scare planning through cooperation with interdisciplinary teams. 
• May rotate into day center services as a CNA as applicable anddeemednecessary by the Director of Clinical Services or Home Care Coordinator.
• May rotate into the clinic as a CNA asdeemednecessary by the Director of Clinical Services.
• May rotate to provide Day Center escort for participants in the community asdeemednecessary by the Director of Clinical Service. 
• Participates in Quality Improvement programs.
• Handles potentially infectious specimens withappropriate biohazardprecautions, and practices universal precautions.
• Consistently cooperates and supportsorganizationin problem solving issues. 
• Participates in continuing education classes and any required staff and training meetings.

• Responsible formaintainingprofessional affiliations and any required certifications.
• Other duties as requested/assigned
• Assure compliance with infection control, personal protection equipment, blood-borne pathogens, hazardous material handling, and fire safety standards.
• Attend staff meetings and participant care planning on a regular basis.
• Maintain current written records,indicatingongoing documentation of services provided, reassessments of changing needs, and participant’s expressed wishes.
• Prepare andsubmittimelywritten reports asrequired.
• Maintain confidentiality of participant information.
• Additionalresponsibilities will be added atmanagersdiscretion, as career growth opportunities and responsibilities are constantly arising in our flexible and start-up environment.

NON-ESSENTIAL DUTIES AND RESPONSIBILITIES

• Attend andparticipatein staff meetings, in-services, projects, and committees as assigned.   
• Adhere to and support the center’s policies, practices, and procedures.  
• Accept assigned duties in a cooperativemanner;and perform all other related duties as assigned.
• Be flexible in schedule of hours worked.
• May require use of personalvehicle.

QUALIFICATIONS

Experience:

• Minimum of one (1)yearsof documented experience working with a frail or elderly population preferred.
• Experience in a long-term care facility or with a community-based geriatric program preferred.

Knowledge, Skills, and Abilities:

• Knowledge of the principles and techniques involved in relating to frail elderlypersons.
• Skill in communicating with participants, their families, and other staff members; ability toestablisha helping, trusting relationship with participants.
• Demonstrates sensitivity to the needs of older adults asevidencedby a caring approach, patience in providing care, and responsiveness to participant requests.
• Ability to follow directions,verbaland written instructions, and adaptworkschedule under supervision.
• Ability to followthrough withinstructions and directions provided by supervisor, RN, QAPI coordinator, primary care physician, and/or medical director.
• Ability toutilizeappropriate safetymeasures when providing care.
• Ability to relate to the public and governmental agencies in a pleasant, patient, and professional manner.
• Ability to work effectively and harmoniously as part of an interdisciplinary team, the elderly, providers of services, andpeers in particular.
• Well organized, dependable, flexible, and resourceful.

Education and Certification:

• Mustpossess, as a minimum, a GED or high school diploma.
• Must be an active certified nurse assistant inMaryland.
