Description
  • Conduct evaluations for patients in both inpatient and outpatient settings, creating personalized treatment plans
  • Administer physical therapy techniques focused on enhancing mobility, strength, balance, and overall function
  • Collaborate effectively with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
  • Provide education to patients and their families about therapeutic aims, discharge strategies, and home exercise regimens
  • Maintain thorough documentation of evaluations, treatments, and patient progress while adhering to facility and regulatory standards
  • Engage in care team meetings and interdisciplinary conferences to ensure seamless coordination of care
Education
  • A Doctorate in Physical Therapy (DPT) or an equivalent accredited degree
Certifications
  • State licensure as a Physical Therapist
  • CPR certification is preferred
  • Specialized certifications, such as neurological or geriatric, are advantageous, but not mandatory
Skills
  • Prior experience in physical therapy is highly preferred
  • Skilled in addressing the needs of patients with complex medical and mobility challenges
  • Excellent communication and teamwork abilities
  • Adept in digital charting and clinical documentation processes
