- Conduct evaluations for patients in both inpatient and outpatient settings, creating personalized treatment plans
- Administer physical therapy techniques focused on enhancing mobility, strength, balance, and overall function
- Collaborate effectively with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
- Provide education to patients and their families about therapeutic aims, discharge strategies, and home exercise regimens
- Maintain thorough documentation of evaluations, treatments, and patient progress while adhering to facility and regulatory standards
- Engage in care team meetings and interdisciplinary conferences to ensure seamless coordination of care
- A Doctorate in Physical Therapy (DPT) or an equivalent accredited degree
- State licensure as a Physical Therapist
- CPR certification is preferred
- Specialized certifications, such as neurological or geriatric, are advantageous, but not mandatory
- Prior experience in physical therapy is highly preferred
- Skilled in addressing the needs of patients with complex medical and mobility challenges
- Excellent communication and teamwork abilities
- Adept in digital charting and clinical documentation processes
