Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients and ensures the design of functional treatment programs for an effective return to community activities. The therapist serves as an educator of patients, families, students, coworkers, and themselves while performing other professional duties associated with patient care and development of therapy services.

Responsibilities

• Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans and goals.
• Documents patient care clearly, accurately, and timely in compliance with organizational policies.
• Ensures care is provided under signed, valid physician orders and within insurance approval.
• Participates in the student program, providing supervision and evaluation.
• Creates an individualized professional development plan and pursues ongoing education.
• Maintains professional licensure in good standing and adheres to ethical standards.
• Implements organizational performance improvement initiatives to enhance service quality.

Required Experience / Certifications / Licensure

• Bachelor's or Master's degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR Certification.
• Active state licensure for practicing as a Physical Therapist.
