We are seeking a dedicated Social Services Director to join our leadership team and advocate for the emotional, social, and psychosocial well-being of our residents.
Position Summary

The Social Services Director is responsible for overseeing the facility's social services program, assisting residents and families with admissions and transitions, coordinating discharge planning, and ensuring residents receive the support and resources needed to achieve the highest quality of life.
Responsibilities

• Complete psychosocial assessments and develop individualized care plans.
• Provide support and counseling to residents and families.
• Coordinate admissions, discharges, and community resource referrals.
• Participate in interdisciplinary care plan meetings.
• Advocate for resident rights and address concerns or grievances.
• Assist with discharge planning and transition of care.
• Maintain accurate documentation in accordance with state and federal regulations.
• Collaborate with nursing, therapy, admissions, and other departments to ensure comprehensive resident care.
• Build positive relationships with residents, families, and staff.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred.
• Long-term care or healthcare experience preferred.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of discharge planning, resident advocacy, and regulatory compliance is a plus.
