Communicates effectively with the patients family or caretakers;/liliParticipates in planning for discharge and coordinates referrals, as appropriate;/liliProvides discharge instructions to patient and significant other./liliEvaluate care provided for patient outcome/liliDemonstrates ability to measure effectiveness of care provided and documents same;/liliPerforms variance analysis related to outcome data for performance improvement;/liliDesigns, implements, and evaluates systems to improve care in Unit;/liliKeeps accurate documentation of patients treatment, activity and condition;/liliUses appropriate methods of documentation according to departmental policy./liliCollaborate with other care team members in planning and carrying out treatment regimen; provides direction to other members of the care team/liliCollaborates with the appropriate physician on patients plan of care;/liliAccurately interprets and implements treatment regimen as prescribed by the Physician;/liliAssists the Physician during treatments and/or diagnostic procedures;/liliKeeps the Patient Care Manager/designee and/or physician, abreast of changes in patients condition and/or treatments, as appropriate;/liliUses clinical judgment in delegating assignments to the Patient Care Associate, Support Care Associate and Clerical Service Associate in providing patient care, and ensures that assignments are completed in a timely fashion./liliPerforms grade I-IV Decubritus Care/liliPerforms preventive skin care measures/liliApplies simple dressings, maintaining principles of aseptic technique;/liliApplies warm cold compresses, consistent with facility procedure;/liliPerforms irrigations, consistent with Facility procedure;/liliPerforms other procedures related to skin care, as necessary./liliAdministers medications correctly and safely/liliCorrectly identifies medication: action; dosage; side effects and implications for the patient;/liliMeets standard on medication administration examination;/liliDemonstrates preparation of local solutions;/liliDemonstrates preparation of I.V. drips;/liliAdministers and documents medication correctly;/liliAdministers IVP medication as per department policy and procedure;/liliEducates patients and significant others related to drug and food interactions./liliParticipates in patient and family education/liliProvides the patient with an explanation of his/her condition as indicated;/liliCommunicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies;/liliRecognizes and utilizes health teaching opportunities and resources /materials available for this teaching;/liliProvides for early discharge planning and appropriate referrals for post-hospital care;/liliEvaluates the effectiveness of teaching by feedback from patient/family and documents same./liliPerforms related duties, as required./liliADA Essential Functions/li/ulh3Job Qualification/h3ulliGraduate from an accredited School of Nursing. Bachelors Degree in Nursing, preferred.
