In this position, you will provide one-on-one Occupational Therapy services to patients in their homes, helping them regain or maintain independence with daily activities after illness, injury, or surgery. Typical responsibilities include performing comprehensive OT evaluations in the home; designing and implementing personalized treatment plans; recommending adaptive equipment and home modifications; educating patients and caregivers on safe techniques, energy conservation, and fall prevention; and coordinating closely with nursing, physical therapy, speech therapy, and medical providers. You will also update the care team on patient progress and adjust interventions based on clinical findings and patient goals.
