Nurse Practitioner - Primary Care

Nurse Practitioner Position

This position is located at the McMinnville Outpatient Clinic in McMinnville, TN. The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties include:

• Independently and responsibly managing a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
• Developing and implementing an individualized plan to achieve optimal population health and disease management
• Demonstrating leadership and working collaboratively with the Primary Care Team and other interdisciplinary team members
• Performing histories and physical exams
• Providing appropriate documentation to minimally include history, physical exam findings, diagnosis, definitive care plan and disposition of the patient
• Ensuring that patient encounters are appropriately closed out including disposition that clearly indicates the next step of care
• Ensuring that each patient is provided with a list of current medications and an appointment list at close-out
• Communicating test results to patients
• Ordering and interpreting labs, tests, and diagnostic studies
• Prescribing pharmacologic and non-pharmacologic interventions
• Initiating referrals and consults to specialty services
• Providing education to patients and their families
• Assessing and adjusting the plan of care as needed to achieve optimal patient outcomes

• Demonstrated experience leading clinical quality improvement initiatives or managing healthcare programs
• Background in project management within a clinical setting strongly preferred
• Training or certification in quality improvement methodologies (e.g., Lean, Six Sigma) is a plus
• Proven ability to implement and sustain process improvements in primary care or outpatient environments
• Other duties as assigned

Pay: Competitive salary, regular salary increases, potential for performance awards
