Job Details Job Location: The Bradford Skilled Nursing and Rehabilitation - SHREVEPORT, LA 71118 Job Shift: Day Social Worker Responsibilities Foster a climate, policies, and routines that enable residents to maximize their individuality, independence, and dignity. Provide patients with the highest practical level of physical, mental, and psychosocial well‑being and quality of life. Identify psychosocial, mental, and emotional needs and assist in developing and providing services to meet those needs. Serve in accordance with the National Association of Social Workers (NASW) Code of Ethics and all relevant Federal, State, and Local regulations. One year of supervised social work experience in a healthcare setting working directly with geriatric individuals. Minimum of two years of nursing experience in a Skilled Nursing Facility (preferred).
