Key Responsibilities

Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established therapy goals
Collaborate with interdisciplinary care teams to support patient outcomes
Ensure compliance with facility policies and applicable regulatory standards

Required Qualifications

Active Physical Therapist (PT) license in the state of Wisconsin
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in a clinical or rehabilitation setting

Preferred Experience / Skills

Prior experience in a Skilled Nursing Facility (SNF) environment
Familiarity with Medicare documentation and compliance requirements
