Responsibilities
- Evaluate residents’ physical abilities and develop individualized treatment plans in the long-term care setting
- Provide therapeutic interventions including gait training, therapeutic exercises, balance training, ROM, and activity of daily living optimization
- Document assessments, progress, and outcomes; adjust plans based on resident response and goals
- Collaborate with the interdisciplinary team to support discharge planning, caregiver education, and safety initiatives
- Ensure patient safety, apply appropriate modalities, and maintain equipment and infection control standards

Requirements
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Active Physical Therapist license in Washington or eligibility for licensure in WA
- Experience in geriatric care or long-term care settings preferred
- Ability to work in a long-term care environment and collaborate with a multidisciplinary team
- Strong documentation, communication, and patient care skills; CPR/BLS preferred
