As a Case Manager , you'll provide comprehensive, trauma-informed case management to individuals and families living in our emergency shelter as well as clients who have transitioned into permanent housing.

You'll get to know each client as an individual, assess their needs and goals, develop personalized service plans, coordinate resources, advocate alongside them, and provide support as they work toward safety, housing stability, and long-term independence.

You'll work both inside and outside the shelter—meeting with clients, conducting home and community visits, coordinating services, building relationships with community partners, and helping clients navigate the challenges that can arise after moving into permanent housing.

This position is ideal for someone who builds trust easily, thinks creatively, stays calm during crisis, and believes that empowerment means walking alongside clients rather than making decisions for them.

• Provide trauma-informed case management, advocacy, crisis intervention, safety planning, and support to residential shelter clients and clients who have transitioned into permanent housing.
• Conduct comprehensive assessments and develop individualized service plans focused on safety, housing stability, employment, education, financial wellness, health, and long-term independence.
• Monitor client progress, evaluate outcomes, and adapt service plans as client needs and circumstances change.
• Support shelter intake, orientation, and client transitions into shelter while helping create a warm and supportive experience.
• Provide ongoing crisis intervention, emotional support, and problem-solving for individuals and families experiencing domestic violence, sexual assault, homelessness, and other crises.
• Conduct individual case management meetings, support groups, and life-skills classes that help clients build skills and work toward their goals.
• Coordinate and provide transportation for clients to appointments, housing, employment, and other essential services, including safely operating agency vehicles.
• Lead house meetings and assist with implementing shelter expectations and daily routines.
• Collaborate closely with Case Aides and other shelter staff to support client goals and maintain a safe, welcoming shelter environment.
• Identify clients who may benefit from continued support after leaving shelter and provide ongoing post-housing case management.
• Conduct home visits, office visits, community visits, and virtual meetings to monitor housing stability and client progress.
• Help clients develop skills related to budgeting, employment, transportation, education, parenting, healthcare, financial management, and independent living.
• Connect clients with community resources, including housing programs, employment services, healthcare and behavioral health providers, legal assistance, childcare, food assistance, and financial literacy programs.
• Advocate with and alongside clients as they access benefits, public assistance, disability services, housing resources, and other supports.
• Build collaborative relationships with landlords, housing providers, schools, employers, healthcare providers, community agencies, and other service partners.
• Maintain timely and accurate case documentation, electronic records, assessments, referrals, service plans, and grant-related documentation.
• Assist with program data collection and grant reporting requirements.
• Participate in staff meetings, case reviews, multidisciplinary meetings, trainings, and professional development opportunities.
• Participate in the agency's Sexual Assault Crisis Response on-call rotation.
• Represent CCCC professionally in the community and support outreach, fundraising activities, and special events as assigned.

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related social services field required; Master's degree preferred.
• Equivalent experience may be considered in lieu of a degree.
• One to two years of experience in case management, housing services, victim advocacy, homelessness services, or related human services preferred.
• Knowledge of trauma-informed care, crisis intervention, social work theory and ethics, domestic violence, sexual assault, homelessness, and housing stability.
• Experience working with unhoused populations and an understanding of the challenges and community impacts associated with homelessness preferred.
• Knowledge of community resources, housing programs, and supportive services available to individuals and families transitioning from homelessness or crisis situations preferred.
• Proficiency with Microsoft Office 365 and electronic client management systems.
• Strong organizational, interpersonal, communication, documentation, and time-management skills.
• Ability to recognize problems, gather information, and develop appropriate solutions using established protocols and best practices.
• Ability to work independently while contributing effectively to a multidisciplinary team.
• Strong written and verbal communication skills, including the ability to facilitate groups and communicate effectively with diverse audiences.
• Ability to communicate in a courteous, client-focused, and trauma-informed manner.
• Bilingual in English and Spanish preferred.
• Current, valid Texas driver's license and proof of auto liability insurance.
• Reliable personal vehicle available for local travel.
• Ability to safely operate agency vehicles, including a van and 14-passenger bus.
• Ability to pass an extensive background check, including fingerprinting, drug screen, and driver's record check.
• First Aid and CPR certification required; may be completed after hire with agency support.
• Ability to work a flexible schedule, including day, evening, weekend, and holiday shifts as assigned, as well as participate in the Sexual Assault Crisis Response on-call rotation.
